Job description:
An interdisciplinary diabetic foot and limb preservation practice in Waterloo, Ontario, is seeking a Nurse Practitioner (NP) to join a specialized clinical team serving as a regional resource for the Region of Waterloo and surrounding communities.
This is an opportunity to work in a focused, high-acuity outpatient setting dedicated to the assessment and management of complex diabetic foot disease and chronic wounds. The successful candidate will work closely alongside a specialist physician with expertise in wound management as part of a collaborative, interdisciplinary care model centered on limb preservation and functional outcomes.
Role Overview
The Nurse Practitioner will provide comprehensive, patient-centered care to individuals with diabetic foot complications and chronic wounds. The role emphasizes clinical assessment, longitudinal management, and patient education.
Key clinical functions include performing minor bedside surgical procedures, such as sharp debridement for wound bed preparation, as well as the advanced pharmacological management of infections through topical therapeutics and systemic antimicrobial prescribing.
Key Responsibilities
Clinical Assessment: Perform comprehensive physical exams and assessments of patients with diabetic foot ulcers and complex wounds.
Advanced Wound Management: Develop and implement evidence-based management plans, including bedside debridement, dressing selection, and monitoring of healing trajectories.
Pharmacological Intervention: Manage wound bioburden and infection via the prescribing of topical therapeutics and systemic antimicrobials.
Interdisciplinary Collaboration: Work closely with the lead specialist physician (Infectious Diseases), community providers, Ontario Health atHome, and allied health services to ensure seamless care transitions.
Specialist Coordination: Act as a key liaison and coordinate care with external specialist physicians in Vascular Surgery and Orthopaedic Surgery to ensure timely intervention for limb preservation.
Patient Advocacy: Educate patients and families on wound care, diabetes management, and prevention strategies to reduce recurrence.
Documentation: Maintain accurate, timely, and thorough clinical documentation in accordance with CNO standards.
Qualifications
Licensure: Current registration as a Nurse Practitioner in good standing with the College of Nurses of Ontario (CNO).
Expertise: Experience in wound care, diabetic foot management, is considered a strong asset.
Clinical Skills: Proficiency in or a willingness to be trained in conservative sharp debridement and specialized antimicrobial stewardship.
Attributes: Strong clinical judgment and the ability to work independently within a collaborative, fast-paced model focused on limb preservation.
Communication: Excellent interpersonal skills for navigating complex specialist referrals and patient education.
