JOB SUMMARY:

Tsi Kanonhkhwatsheríyo IIPCT is dedicated to enhancing healthcare within our communities

through collaborative care models and integrated services. Our mission is to provide patient-

centered care that is accessible, comprehensive, and culturally sensitive. We are currently seeking

a dedicated and compassionate Nurse Practitioner to join our team in our mobile medical

clinic. As a Nurse Practitioner, you will be a primary healthcare provider responsible for

delivering comprehensive, longitudinal care to a diverse patient population. You will play a

critical role in promoting health and wellness, managing chronic conditions, and providing

preventive care within our communities. This position offers a unique opportunity to work in

Indigenous Peoples within our catchment.

Key Responsibilities:

• Primary Care Services: Provide comprehensive primary care, including diagnosis,

treatment, and management of acute and chronic illnesses wwithin the scope of practice as

defined by the College of Nurses of Ontario (CNO), the Nurse Practitioner (NP) will

provide comprehensive primary care services to clients of all ages; this includes primary

prevention, health education, comprehensive physical exams, chronic disease

management, antenatal care, and acute/episodic visits

• Visits take place predominantly in the mobile clinic setting but may also include telephone

contact and virtual visits as required

• Review client laboratory results, procedure reports and consultation notes and arrange

appropriate follow-up/referrals as required

• Documentation of all client contact, both direct and indirect

• Ensure that all client-based information is always kept confidential

• Consultation with physician partner regarding client issues that are outside the NP scope

of practice

• Patient Assessment: Conduct thorough health assessments, including physical exams and

health histories.

• Care Planning: Develop and implement individualized care plans, focusing on preventive

health, chronic disease management, and patient education.

• Collaborative Care: Work closely with physicians, nurses, and other healthcare

professionals to coordinate care and optimize patient outcomes.

• Collaborate with community partners to offer outreach services as required

• Teaching and training of team members and student placements on an occasional basis as

required

• Community Engagement: Participate in community health initiatives, educational

programs, and outreach activities to promote health and wellness.

• Documentation: Maintain accurate and timely documentation of patient care, adhering to

professional standards and regulatory requirements.

• Advocacy: Advocate for patients within the healthcare system, ensuring equitable access

to healthcare resources and services.

• Additional duties assigned by the Executive Director as required

EDUCATION, EXPERIENCE, KNOWLEDGE, SKILLS AND ABILITIES

REQUIREMENTS:

• Current registration is in good standing with College of Nurses of Ontario as a Registered

Nurse in the Extended Class [RN(EC)]

• Experience in Indigenous or rural healthcare is an asset.

• Knowledge of Indigenous culture, traditions, ceremony and connecting clients to cultural

practices that can be used in supporting clients.

• Ability to work and interact with patient care professionals as part of an interdisciplinary

team

• Strong interpersonal, communication, organizational and clinical skills.

• The ability to work and relate effectively with a wide range of constituencies in a diverse

community

• Demonstrate patience, discretion, a high level of professionalism, high ethical practices,

standards & confidentiality

• Proficient use of MS Office, Outlook, and Electronic Medical Records applications

• Must be willing and able to travel to other communities within the catchment area to provide

services

• Must be able to provide a clear Vulnerable Sector check

• Must have current CPR/First Aid and AED certification

• Must have a valid ‘G’ Driver’s License, Clear Driver’s Abstract
